Leslie Walter Jacobson, M.D., F.A.C.S., age 86, born in Minneapolis, Minnesota, November 5, 1931, passed away peacefully on September 17, 2018. Preceded in death by father, Walter Jacobson and mother Grace Jacobson. Survived by wife, Catherine (Cathy), children Brad (Tamara) Jacobson, Mary Jacobson, Sally (Mark) Peterson: grandchildren, Alex, Olivia, Nick, James, Nina, Charlie & Henry: great granddaughter, Olivia.
Les was a graduate of the University of Minnesota, receiving a Bachelor of Arts degree, Bachelor of Science degree and his Medical degree. After a general internship at Hennepin County General Hospital in Minneapolis, he spent 2 ½ years as a Captain in the U.S. Army, both overseas and domestically and was awarded the Army Commendation Medal. He returned to the University of Minnesota, where he completed a Fellowship in Ophthalmology. He was Board Certified in Ophthalmology and was a member of the American Academy of Ophthalmology, Minnesota Academy of Ophthalmology, American College of Surgeons, American Medical Association, Minnesota State Medical Association, Hennepin County Medical Society and the American Society of Cataract and Refractive Surgery. He was on the teaching staff of the Department of Ophthalmology at the University of Minnesota as an Adjunct Professor of Ophthalmology. He served on the medical staffs of Abbott-Northwestern Hospital, Phillips Eye Institute and Fairview Southdale Hospital.
He started his private practice at Southdale Eye Clinic/Ophthalmology Associates, LLP in June of 1965 and retired in July 2012.
“Jake” as many of his friends commonly called him had many avocations, which included flying his own aircraft (having obtained both his instrument and land and sea ratings), hunting and fishing, downhill skiing and extensive traveling with his wife of 60 years, Cathy and their children.
The family would like to thank the many aids, nurses and physicians involved in his care. They were an endless source of support and comfort.

Funeral Service Wednesday September 26, 2018 11:00 a.m. at Mt. Olivet Lutheran Church, 5026 Knox Avenue South Private interment Fort Snelling National Cemetery. Visitation Tuesday 5-7 p.m. at Gill Brothers Funeral Home, 5801 Lyndale Avenue South and one hour prior to service at church.
